As a credentialed member of the VA Community Care Network, Entrust Senior Care works directly with VA care coordinators to bring authorized, in-home support to eligible veterans — often at little to no cost to the veteran or their family.

Eligibility

Who qualifies for VA in-home care

VA-Enrolled Veterans

Veterans already enrolled in VA health care, or referred by a Veterans Service Coordinator, generally qualify for VA-authorized home care.

Honorable Separation

Veterans separated from military, naval, or air service under conditions other than dishonorable.

Activated Reserves & National Guard

Current or former Reserve or Guard members activated under federal orders (outside of training), who completed their full period of active duty.

Help With Daily Living

Veterans needing assistance with everyday activities — bathing, dressing, grooming, mobility, toileting, and more — are typically the best fit for VA in-home care.

How to apply for VA in-home care

If you're already enrolled in VA health care, here's how the process works.

Contact Your Care Coordinator

If you already have a VA care coordinator, start there — they can confirm your eligibility and begin the authorization process.

Schedule a VA Primary Care Visit

No care coordinator yet? Book an appointment with your VA primary care physician for an assessment of your in-home care needs.

Get Your Referral

If your PCP identifies a need, they'll refer you to a VA caseworker, who coordinates with us to get care started.

Not yet enrolled in VA health care?

You'll need to complete VA Form 10-10EZ to enroll. You can apply online at va.gov, or download and mail a paper copy — either way, you only need to submit it once. Call us any time and we're happy to point you in the right direction.
